sanitary conditions, daily grooming and any personal care you may need for your home to include
but not limited to trash, mail, watering plants etc.

I am hiring PSU to care for my animals and do hereby waive and release Pet Solutions Unlimited from any
and all liability of any nature. This includes injury, death, sickness or damage my pet may cause while in Pet
Solutions Unlimited’s care.

READ CAREFULLY
24 hour set-up fee: An additional fee of 50% will be applied to your invoice for services set-up with less than
24 hours notice. This fee is for visits on the same day and next day.
Cancellations fees: Cancellations without 24 hour advanced notice will be charged fees for one day’s
services. These fees are necessary due to rescheduling and forfeit of business due to Pet Solutions
Unlimited’s commitment to your pets.
Holiday Fees: The following Holidays are subject to additional fees New Years Eve, New Years Day,
Memorial Day, Easter, Independence Day, Labor Day, Thanksgiving, Christmas Eve and Christmas day.
Additional fees are based a minimum of $10 for Cat and $15 for dogs for daily visits.  Holiday rates for
overnight care is 4x your daily rate.
